Overview
Join us to explore how one large integrated care delivery system used a multidisciplinary approach to improve its system‑wide readmission rate. Readmission rates place significant strain on patients, caregivers, and health systems. They disrupt recovery, increase suffering, drive up costs, and are often linked to preventable issues such as gaps in understanding the discharge plan.

At the conclusion of this activity, the learner will be able to verbalize why readmissions are a significant issue and describe several factors and approaches that can contribute to reducing avoidable readmissions.
