AAACN has long been the recognized leader in advancing the practice, education, and research of Care Coordination and Transition Management (CCTM) for Registered Nurses in ambulatory care settings. Now, we’re inviting nurses and nurse leaders across all settings to help guide the next chapter of this important work.

Recently, the Publications and Online Education Review and Recommendations (POERRS) Task Force distributed a brief survey to gather insights from nurses who provide, oversee, or educate others in CCTM practice. We have decided to extend the deadline for this survey; while the initial responses have been extremely valuable, extending the timeline will help us gather additional meaningful data that better reflects the full range of CCTM practice across settings

Your feedback will directly inform decisions about the future of AAACN’s CCTM resources—including potential updates to existing publications and courses, development of new materials, or retirement of outdated offerings.

Your input remains invaluable in ensuring our educational resources reflect current practice and continue to support nurses in delivering safe, coordinated, patient-centered care across the continuum.
